Reasons for Founding the Italian Association for Electroconvulsive
Therapy (italiano)
In Italy, electroconvulsive
therapy ( ECT ) is highly stigmatized by the media and its use,
with very few exceptions, is ostracized by nearly all public hospital
and University psychiatric wards. The local and state health administrations
are clearly against its use. The consequences of this stigmatization
on behalf of the public opinion and the psychiatric community’s
unwillingness to administer electroconvulsive therapy have had a
dramatic effect on many psychiatric patients who suffer the most
from severe illnesses. Experienced psychiatrists are fully aware
that ECT can cure most drug-resistant depressions and that it is
extremely effective in the treatment of Depression with Psychotic
Symptoms, which is also associated with the highest risk of suicide.
Moreover, many chronic bipolar cases can only be treated effectively
with ECT. Bipolar mixed states respond better and faster to ECT
than any other known treatment. Some severe, life-threatening manic
states do not respond to pharmacotherapy whereas they do respond
quickly to ECT. Catatonic states of any origin are best cured with
ECT. Furthermore, ECT is the only way to treat severe Pregnancy
Depression without causing any damage to the fetus.
In Italy, this worrisome
situation has continued to deteriorate primarily because of ideological
prejudice, without taking into account the results of clinical trials
and the risk-benefit ratio. Denying access to ECT causes long-term
suffering in many patients and their families, has contributed to
many suicides that might have otherwise been avoided if ECT had
been administered in a timely manner, and dramatically increases
the cost of psychiatric assistance.
According to Fred K.
Goodwin: “Lack of access to a well-documented therapy –
sometimes the only available – is a cruel outrage to society
and should be considered unacceptable”. The Italian Association
for Electroconvulsive Therapy will contribute to the research, information
and training of psychiatrists in the clinical use of ECT in a wholehearted
attempt to overcome this intolerable situation.
